972-993-7498Our team of providers consists of physicians who are Board Certified in Pain Medicine, Physical Medicine & Rehabilitation, and Addiction Medicine. Our physicians and mid-level providers hold certification in Pain Management and are Buprenorphine Waivered providers.
We work with the facility to ensure compassionate, compliant care to residents. We collaborate closely with DONs, ADONS, nursing staff, MDS coordinators, social workers, and admitting primary care team.
Our team will work with the admissions team to get the medications electronically sent to the facility pharmacy promptly, so the patient and nursing staff are prepared to address the patient’s pain needs upon arrival. We review the discharge medications, along with any notes the nurse can provide, and check the required state PMP prior to electronically sending the medications to the pharmacy.
Each patient is seen by a mid-level provider and physician within 72 hours at the facility so a complete consult note can be sent to the MDS coordinator.
